Patricia Castillo has lost nearly 40 pounds two months after gastric sleeve weight loss surgery.

Two months ago, I had gastric sleeve weight loss surgery.

Now that I’m down 37 pounds, there are five things I’ve learned about myself:

  1. Sugar was definitely an addiction.
  2. It wasn’t the three cups of coffee I wanted, but the sugar and creamer.
  3. I didn’t just have acid reflux that kept me up at night. I had a severe hiatal hernia that wasn’t discovered until I went for pre-op testing. That is what caused the acid reflux!
  4. I was dehydrated! Between all the coffee and drinking very little water, my fatigue was more from dehydration.
  5. The idea of eating protein first is changing my life. Pre-op life was eating carbs first.

I also realized that all the exercise in the world didn’t matter when I had all these pre-surgery barriers weighing me down… literally.

COVID-19 – My bariatric pivot

The COVID-19 pandemic resulted in me suddenly working from home and required me to make a choice.

How should I handle the stress of working from home and the general stress brought on by living through a global pandemic?

Should I resort to the comfort of old habits and begin to stress eat again?

No way. I have come too far.

I decided to take advantage of this time at home and tools I was given by my Prime Surgicare team:

  • I continue to meal plan as well as weigh and measure my meals.
  • I decided to increase my activity to reach 10k steps a day. That means a lot more walks outside with my dog and 30-minute dance workout videos at home.

Moving forward during my ‘new normal’

I still have 53 more pounds to lose. The difference now is that losing 53 pounds actually feels doable.

Bariatric surgery was a much-needed system reset for my body. The operation helped to diagnose my hiatal hernia, repaired during my bariatric operation, so my chronic acid reflux is a thing of the past (good riddance) along with trying to sleep sitting up all night.

I no longer need coffee to boost my energy level just to get through the day.

Weight loss surgery, and the new life tools I have learned, will carry me on a healthy journey for the rest of my life, even during these incredibly stressful and dangerous time.

Yes, medical insurance covers telehealth visits.

What if I have Medicare?

Medicare released a statement confirming they will cover telehealth and phone visits.

What if I don’t have medical insurance?

Self-pay patients will pay a consultation fee of $125.00 per visit.

Does a telemed visit cost the same fee as an office visit?

Yes, the fee for live and telemed visits is the same.

How do I pay my co-pay?

We offer two options:

  • You can quickly pay your co-pay to our Prime Surgicare PayPal account, if this fails, we can send you a printed invoice through the USPS and you can write a check and mail it back.

What days/ times can I make an appointment?

Appointments with our staff are available Monday through Friday, beginning at 9 am until 4:30 pm. Call the office to schedule your appointment and we will email you a link to click on five minutes before your appointment time.

We will also make every effort to offer after-hours appointments for COVID-19 essential workers.

How long does it take for me to get an appointment using Telemed?

Appointments are available now within 1-2 weeks depending on the provider

What is the telemed appointment cancellation policy?

Please make every effort to cancel or reschedule your appointment 24 hours in advance. We know emergencies happen, especially now, and will not penalize our patients for last-minute cancellations.

Are any in-office appointments available?

To secure the safety of patients and staff, we will not have any in-office visits until further notice.

What should I do if I have a medical emergency?

If you are experiencing a medical emergency, call 911 or proceed to the Emergency Room. If the emergency is of a surgical nature, or related to an existing surgical condition, please also contact our office promptly.

By now, you know the phrase “social distancing‘ as well as you know your own name.

But how does it help us slow the spread of COVID-19?

When you choose to socially isolate, you are playing an incredibly important role in helping to overcome the spread. In my latest YouTube video below, I explain the three essential benefits that your family’s social isolations offers:

1) Protecting your family against exposure to the virus.

In my last video on social distancing, https://youtu.be/WRBWVgzvzWE, I explained that the most dangerous carriers of COVD-19 are those with no symptoms yet, who are still out in public touching surfaces and mingling with others.

2) Inhibiting the spread of the disease.

If any of your family members are (unknowingly) infected, you are not spreading the virus outside your home.

3) More specific identification of others exposed to the virus.

By limiting your exposure to outsiders, there are fewer people you may have exposed to the virus; it also makes it easier to reach out to these people and alert them that they are exposed and should immediately self-isolate for 14 days.

Finally, we have a finite number of hospital beds, infection control products, medications and clinicians who can optimally treat patients with coronavirus.

When your family stays safe through social distancing, you are freeing up services and supplies for those who have been diagnosed.

Social isolation prevents you from spreading the virus and defends you against contracting it from someone else. Win, win.

The COVID-19 pandemic has spread rapidly for a few key reasons. One of them is the fact that infected people may have no symptoms of the disease for FIVE DAYS or more.

The virus is spread through tiny, undecipherable droplets that come from the mouth and nose, often through just speaking, laughing or exhaling.

You can also contract the virus by touching a surface where one of these droplets has settled, and later touch your face in some way.

Therefore, the 6-foot social distancing protocol is designed to protect you from contracting these droplets from people who seem sick–and, more importantly, those who look 100% healthy, but may be in the early stages of the virus, unknowingly.

I’m encouraged that the CDC and the New Jersey Department of Health pushed social distancing early-on.

This is actively decreasing the additional spread of the virus now, rather than in the future, when the government may have no choice but to mandate isolation. Social isolation prevents you from spreading the virus and defends you against contracting it from someone else. Win, win.

Following the Centers for Disease Control (CDC) protocol to protect the public against COVID-19, and prioritizing the safety of our patients and staff, we have made the following operational changes effective Monday, March 16, which will remain in effect until at least Monday, March 30, 2020.

Discontinuing patient office hours

Our office will remain open from 9 am to 5 pm via phone and email communication only. The building is not open to patient traffic of any kind. Unless all phone lines are in use should you call, a staff member will answer your call, or return your call within 30 minutes.

Post-op patients

Post-op patients, those who had surgery from March 9 to March 13, will be contacted personally on Monday, March 16.

All other patients requiring follow-up care who have non-emergency questions, concerns or potential complications, call the office at (732) 982-2002 or email us at info@primesurgicare.com. (Emails will be answered within 24 hours).

Should you experience a life-threatening emergency, dial 911.

Please do not send clinical (medical-related) messages via social media, as this can delay a timely response to your issue.

We will announce when in-office visits will resume and invite you to call the office to schedule your next visit.

Elective surgeries

All scheduled surgeries of a non-emergency nature are postponed and will be rescheduled as soon as possible.

In the interim, should you have a pre-op emergency, call our office and report to the Emergency Department. If your symptoms are life-threatening, dial 911.

One of the most critical health concerns that worries me is the debilitating effect chronic stress has on my patients’ emotional and physical health.

Since the majority of the people I treat are battling excess weight, stress can also play a role in inhibiting successful weight reduction.

It doesn’t matter what time of year it is, how old you are or how much money you have in the bank—everyone has to live with stress.

Looking for stress release in all the wrong places

Our ancestors ate solely for the purpose of survival. Today, we eat for a multitude of reasons, many having nothing to do with genuine hunger or the need to fuel the body in order to perform.

Many of the wrong reasons we eat are due to the mind and body looking for a resource to reduce the stress and bring on inner calm.

We’re sometimes drawn to the wrong things to lessen stress—tobacco, alcohol, narcotics and—food.

Stress is a key trigger that causes the most trouble for people battling metabolic disease (obesity).

Many of my weight loss surgery patients live with high levels of chronic stress, whether it’s from workload, relationship challenges or to-do lists that are longer than Route 9.

How stress can threaten your weight loss

Our bodies respond to this stress the way our ancestors’ bodies did: triggering “fight or flight” chemicals in the brain that lead to calorie accumulation and fat storage.

But the difference is that we have plenty of food at our disposal; they didn’t. So we end up continually upgrading the size of our storage unit. Before you know it, you are shopping at the Big and Tall shop.

When you have chronic stress, your body steps up its production of cortisol and insulin.

Your appetite increases, and so do the chances you’ll engage in emotional eating in the form of “comfort foods” like addictive empty-calorie sweets and fats.

As a result, you store more fat, pumping even more of it as well as inflammatory chemicals into the liver.

When you’re stressed, reach out to foods and activities that will provide a steady state of satisfaction so that you never experience those massive hormonal highs and lows that make you search for good-for-your-brain, bad-for-your-waist foods.

Here are some stress reduction tools I share with my bariatric patients, even after they’ve achieved their goal weight:

Six Tips to Get the Stress Off Your Back

 

  1. Protein-rich foods like beans, eggs and Greek yogurt can increases serotonin, improving your mood and combating depression. They also help you reduce your appetite for simple carbs.
  2. Fish and most nuts are rich in omega-3 fatty acids, which have long been known as brain boosters and cholesterol clearers.
  3. Green tea contains catechins, thought to inhibit the breakdown of fats as well as the production of an inflammatory substance that can trigger hunger. One study shows that drinking three big mugs of green tea a day can help you reduce body weight, waist circumference and boost metabolism.
  4. When you’re stressed, your quality of sleep I advise patients to exercise even more during tough times. It helps calm you down and enhances the ability to sleep better. You may not sleep through the night, but you’ll be significantly less exhausted the next day.
  5. Refresh your food routine. You probably have a list of healthy foods you eat regularly. Rather than deal with the temptation of eating the wrong foods, widen your food horizons and visit a different grocery store or restaurant to discover some new options. These items will feel like ‘treats’ and focus your mind away from empty calorie choices. If you always shop at Stop & Shop, visit Trader Joe’s or Wegmans for a fresh look at fresh food.
  6. Leave stress at the door or office and get out to exercise, no matter what. Devote just 30 minutes to fast walking, biking, lifting weights, swimming or taking a Zumba class. (If you’re my patient, you have probably already experienced the good feelings derived from boosting your heart rate). Use these good feelings to combat the stress, even for a few minutes.
